A few things about this Chaplains Report Form. It is a simple form that helps with a lot of things. Such as:

- It helps Membership, if we are paying for members and they have passed, it gets our membership numbers accurate.
- It helps keep records in order.
- It helps to keep up with the Memorial Book for Convention.
- It helps with reporting.
